Better Care Fund

 The "Ealing Connect" community outreach consortia project is lead by SCA and involves Southall Day Centre, Acton Community Forum and Centre for Armenian Information and Advice. 

Our campaign will promote wider NHS use and help people understand about self care options, strongly involving older BME residents (ie Indian, Pakistani, Armenian, Afghan, Somali, African-Caribbean etc) from the most deprived wards of Ealing. 

Our key project aims are to :

&bull ;enable older BME residents to stay out of hospital/remain independent (e.g. reducing falls, health anxiety)
• improve understanding of preventative services
• develop community outreach programme that improves understanding of health services: pharmacies, GPs, 111, Urgent Care Centres

The project will have three distinct phases:
